About The Position

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Bloom | Good Shepherd Women's Health. Requirements

Oklahoma Licensed Professional Nurse (LPN) License. Must have a current and valid LPN license in the state of Oklahoma.
Effective Communication. Strong ability to communicate with diverse individuals, including patients, families, and healthcare professionals.
Problem-Solving and Critical Thinking: Demonstrated skills in problem-solving and critical thinking to provide the best patient care.
Respect and Compassion: Deep respect for the dignity of all patients and their families/caregivers.
Technical Proficiency: Excellent computer skills for efficient internal communication and documentation.
Team Collaboration: Ability to work independently and collaboratively within a team environment.

Responsibilities

Conduct comprehensive patient assessments and develop collaborative treatment plans.
Promote patient empowerment and self-determination; assist with adherence to treatment plans and self-management goals.
Monitor, evaluate, and document patient progress.
Collaborate with outside service providers to coordinate patient treatment and manage referrals.
Record all interventions in patient charts and produce service delivery and outcome reports.
Communicate through personal contact, phone, email, and regular mail; return messages promptly.
Work with Chief Operations Officer and Clinical Director on data collection for outcome measurement reporting.
Report patient progress in files and to medical staff.
Collect and prepare laboratory specimens; coordinate with lab companies as needed.
Perform basic laboratory and point-of-care testing.
