About The Position

WCWC Licensed Practical Nurse (LPN) The Licensed Practical Nurse (LPN) will play a crucial role on the care teams, operating in a remote capacity. In this role, LPN will manage Electronic Health Records (EHR) and patient portals. In addition, the LPN will provide patient education, troubleshoot Remote Patient Monitoring (RPM) equipment and results, counsel patients with chronic diseases, follow up with patients in between provider visits, be a provider extender, and refill medications per protocol. The LPN will follow established protocols in order to facilitate effective communication among healthcare professionals and patients, ultimately enhancing patient outcomes and relieving provider stress.

Requirements

  • Current Licensed Practical Nurse license in Louisiana.
  • Graduate of an accredited practical nursing program.
  • Current BLS certification.
  • Experience in women's health, OBGYN, pediatrics, family medicine, or community health.
  • Basic experience with electronic health records.
  • Licensed Practical Nurse Certificate.
  • Minimum of 2-4 years of full-time clinical experience as an LPN, particularly in primary care for adult, pediatric and geriatric patients.
  • Experience using at least two electronic health record (EHR) systems is required
  • An active unencumbered e-NCL or Enhanced Nurse Licensure Compact license, additional non-compact state licensure would be considered an asset; candidates should also be willing to obtain additional licenses at the company's request.

  • Knowledge of primary care nursing, care management principles, chronic disease management, and preventive care.
  • Skill in patient assessment, patient education, and coordination of care within an interdisciplinary team.
  • Ability to use electronic health records and standard office software.
  • Strong written and verbal communication skills and attention to detail.
  • Demonstrated commitment to health equity, cultural humility, and trauma informed care

Nice To Haves

  • Experience in OB/GYN, Orthopedic, Ambulatory Care or Home Health are also preferred.
  • Spanish or Vietnamese bilingual skills preferred but not required.

Responsibilities

  • Provide nursing support for women's health, prenatal, gynecologic, and pediatric visits within LPN scope of practice.
  • Room patients, collect vital signs, and document medical history and chief concerns in the electronic health record.
  • Assist providers with exams and procedures including pelvic exams, prenatal visits, Pap tests, contraception services, and pediatric well child visits.
  • Administer medications, vaccines, and injections as ordered.
  • Collect and prepare laboratory specimens such as blood, urine, and swabs.
  • Provide patient education on prenatal care, reproductive health, contraception, breastfeeding, child development, and preventive care.
  • Monitor patients after medications or immunizations and report concerns to providers.
  • Support management of common chronic conditions and preventive services such as immunizations and screenings.
  • Coordinate care with providers, behavioral health team, care coordinators, and community health workers.
  • Assist with referrals, follow up appointments, and transitions of care.
  • Document patient encounters accurately in Athena EHR and follow clinic protocols and regulatory requirements.
  • Participate in quality improvement efforts including immunization tracking and preventive care measures.
  • Use interpreters and language services when needed and provide culturally responsive care.
  • Participate in occasional community outreach or health education events.
