GENERAL PURPOSE: Provides expert patient consultation in areas of expertise and certification for those with GU/GI stomas, wounds, fistulas, drains, pressure injuries. Identifies patients at risk for pressure injury development, recommends preventive care, and is involved in development of evidence-based educational programs for staff and patients/families. Educates/coaches patients/family and staff regarding ostomy care. Supports QI/QA, research, and product analysis. FT position, primarily days, primarily 8-hour shifts. May include some weekend and off-shifts. Acts as a wound and ostomy consultant across all services, responding to requests via e-record. Is an expert clinical resource for nursing staff and collaborates with unit leadership teams to establish unit-based skin/wound and ostomy specialists. Develops evidence-based educational programs for nurses and patients to ensure standards of care are met for prevention and treatment of wounds. Provides mentoring for unit-based skin/wound teams. Pre-operative marking and teaching for adults with planned stomas (may also include pediatric population). On-going teaching for patients with ostomies after surgery. Works closely with surgical teams in assuring standards of care for all ostomy patients. Works with patients, nursing staff and providers to manage problems/issues related to incontinence. Works closely with other clinicians, Materials Management Dept, and nursing unit leadership teams to ensure appropriate use of therapeutic pressure redistributing mattresses, bed systems and select offloading devices. Sits on hospital committees/work groups as needed to represent wound and ostomy care needs and to ensure technology is integrated appropriately into internal services. Excellent organizational, communication and teamwork skills are essential.
