About The Position

The WISEWOMAN High Risk Care Coordinator provides risk reduction counseling and care coordination to program participants at risk for or diagnosed with cardiovascular disease. This role supports patients through motivational interviewing, behavioral change strategies, educating patients on healthy lifestyle practices, and connecting them to appropriate support services to improve health outcomes. The ideal candidate is compassionate, detail-oriented, bilingual, and comfortable working in a structured, high-performance environment while supporting a diverse population.

Requirements

  • Ability to maintain confidentiality and comply with HIPAA regulations
  • Ability to administer the WISEWOMAN program in accordance with program policies, procedures, and CDC guidelines
  • Ability to guide participants through prescribed clinical treatment plans to achieve positive outcomes
  • Flexibility in work assignments and ability to function as a team player
  • Professional, compassionate approach when serving vulnerable populations
  • Willingness to learn and maintain working knowledge of: WISEWOMAN & WHC services
  • Access to Healthcare Network (AHN) services
  • Community and social service resources
  • Spanish bilingual required
  • High school diploma required; Associate degree preferred
  • Minimum 1 year of case management or related experience preferred
  • Minimum 1 year of healthcare or related experience preferred

Nice To Haves

  • Certified Community Health Worker (preferred or must obtain within 90 days of hire)
  • Healthy Heart Ambassador (HHA) certification (preferred or must complete within 90 days of hire)

Responsibilities

  • Provide telephonic and in‑person counseling to program participants
  • Educate participants on cardiovascular disease prevention, lifestyle modification, and disease management
  • Coordinate care and assist participants with healthcare system navigation
  • Refer participants to appropriate medical, behavioral health, and social support services
  • Track participant progress using case management standards, forms, and documentation tools
  • Maintain accurate and timely data entry in the Med‑IT system
  • Monitor program metrics, compliance, and outcomes
  • Process claims and follow up on incomplete claims or visits requiring clarification
  • Update MDEs as required
  • Conduct provider education and maintain collaborative relationships with healthcare partners
  • Perform administrative and program support duties as assigned
  • Participate in staff meetings and regular check‑ins with management
