About The Position

The Williams Care Manager – Assessment Clinician is more than a clinician—you are a connector, advocate, and change agent. In this role, you’ll guide individuals through one of the most important transitions of their lives, coordinating care, conducting assessments, and collaborating across systems to ensure long-term success.

Responsibilities

Lead and coordinate transitions from nursing facilities to community placements
Develop and implement individualized, person-centered transition plans
Collaborate with multidisciplinary teams including medical providers, housing specialists, and recovery supports
Conduct comprehensive clinical assessments
Provide ongoing clinical services before, during, and after transition
Apply trauma-informed, strengths-based, and recovery-oriented practices
Serve as a key liaison between nursing facilities, community providers, and state partners
Represent the agency in case reviews, meetings, and system discussions
Build strong partnerships to ensure continuity of care
Complete and submit all required reports accurately and on time
Track consent decree benchmarks and ensure compliance
Maintain high standards of documentation and accountability
Participate in trainings and program development efforts
Support data tracking, outcomes reporting, and quality improvement initiatives
