About The Position

Cornerstone Family Healthcare is actively recruiting for a Registered Dietician to join our growing team in Harriman. RATE OF PAY/SALARY: $35.00 per hour WORK LOCATION(S): Harriman, NY STATUS: Full-Time Ability to work two remote days after successful completion of training. Requirements

  • Bachelor’s degree (or higher) in Nutritional Sciences, Community Nutrition, Clinical Dietetics, Foods and Nutrition, or a related field from a regionally accredited college or university, or one recognized by the New York State Education Department.
  • Registered Dietitian (RD) credential, or eligibility for registration through the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.
  • Completion of an accredited dietetic internship program is required.

Responsibilities

  • Conduct a complete nutrition assessment, determine nutrition risk, and enroll participants.
  • Conduct breastfeeding assessment and provide participant centered counseling that helps mothers initiate breastfeeding, establish milk supply, exclusively breastfeed, and continue breastfeeding.
  • Address common breastfeeding concerns and issue breast pumps.
  • Develop and implement individual care plans (ICPs) and oversee high risk care to all participants.
  • Refer high-risk participants to other health-related and social services, as necessary.
  • Prescribe, tailor and authorize food packages based on participants’ needs.
  • Reissue food packages.
  • Provide and document participant-centered nutrition education/counseling for participants.
  • Ensure the efficient flow of the assessment and education processes.
  • Track high-risk participants’ progress in improving their health and document for outcomes.
  • Schedule participants secondary nutrition education appointments and high-risk contacts.
  • Participate in in-service training programs for professionals and paraprofessionals.
  • Perform other related duties as assigned by the Site Supervisor and Program Director.
  • Attend and participate in monthly department, All Staff, and other required meetings.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family HealthCare Policy and Procedure Manual and Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financial, and employee relations.
  • Perform other duties as assigned.
