About The Position

Health Imperatives (HI) is a multi-service organization dedicated to reducing health and wealth disparities while advancing meaningful opportunities and economic security for people south of Boston. Our mission is to improve the health and well-being of low-income or vulnerable families and individuals in Southeastern Massachusetts. The communities we serve, including Brockton, New Bedford, Plymouth, the South Shore, South Coast, Cape Cod, Martha’s Vineyard, and Nantucket, have some of the highest poverty rates and largest wealth gaps in Massachusetts. For nearly 50 years, we have addressed the complex needs of people who are disadvantaged by poverty and discrimination and have fallen through the cracks of mainstream systems of care. We provide a wide array of trauma-informed health care, emergency services, and support. Under the supervision of the Program Director, the WIC Family Support Coordinator will serve as a Case Manager for high-risk WIC participants who require additional support for referrals and follow-up for social service needs other than routine WIC services.

Requirements

  • Bachelor’s degree with a major in social work, public health, education, psychology, counseling, or human services. Experience may be considered in lieu of degree for a maximum of two years of the required experience.
  • Minimum of 2 years of experience in providing counseling services or engaged in case management activities.
  • Previous experience in the field of substance abuse, smoking cessation, domestic violence, housing, counseling, or other social service issues.
  • Experience in community partnerships and outreach, advocacy activities, and/or training.
  • Ability to work effectively with diverse cultural groups and socio-economic backgrounds.
  • Computer skills required - Word, Excel.
  • Travel is required. Must have a current and valid Massachusetts Class D Motor Vehicle Operator’s license or equivalent from another state.

Responsibilities

  • Provide counseling and referrals for support services.
  • Complete intake and assessment process on all potential clients, utilizing appropriate forms to track and report referrals.
  • Ensure ongoing follow-up to ensure the receipt of services, as needed.
  • Coordinate with WIC Program Director, community coordinator and Senior Nutritionist to develop a triage system to assist participants in addressing issues or concerns outside of nutritional needs.
  • Maintain WIC confidentiality policies.
  • Work collaboratively with other area resources in empowering clients to access services leading to self-sufficiency.
  • Coordinate with the WIC Community Coordinator to identify community referral resources.
  • Provide ongoing support to satellite sites.
  • All other duties assigned.
