About The Position

Responsible for coordinating and executing external, community outreach, health awareness education, workforce development and building/cultivating relationships with CBO/FBO, faith leaders, small-business owners, schools. Design and develop programs with community leaders. Ability to travel across New York State, including evenings/weekends for community events.

Requirements

• Master's degree in Public Health
• 3 years of experience
• 1 year experience supporting patients in understanding chronic disease management and treatment plans in primary care and research settings
• 1 year providing education of clinical protocols and lifestyle modifications tailored to patients' literacy levels
• 1 year collaborating with medical teams and support staff to coordinate outreach strategies grounded in community need and trust-building
