About The Position

The West Side Regional PrEP Navigator will provide essential PrEP services across various health centers in Cook County, focusing on HIV prevention and education. This role involves assessing health education needs, performing counseling, and facilitating access to PrEP services for individuals at risk of HIV. The navigator will work closely with patients to develop personalized health education plans and ensure they receive the necessary support and referrals to maintain their health and well-being.

Requirements

  • Minimum of a high school diploma or equivalent required; Bachelor's degree in a health-related field preferred.
  • One to three years of experience as an HIV and PrEP related counselor or educator preferred.
  • Bilingual in Spanish and English required.
  • Valid Driver's License and car insurance required.
  • Strong oral and written communication skills.
  • Strong computer skills, including experience with Microsoft Excel.

  • Experience with multiple HIV rapid testing preferred.
  • Ability to work with a culturally, racially, and ethnically diverse population.
  • Ability to work within a multidisciplinary team.

Responsibilities

  • Provide patient-centered HIV counseling and testing, PrEP education, assessment, and referrals to CCHIP Clinic and throughout Cook County Health facilities.
  • Perform pre and post HIV counseling and testing with diverse populations, including adults, adolescents, and children.
  • Conduct rapid HIV testing and assess participants for PrEP services following CDC guidelines.
  • Collect and enter data on HIV testing, PrEP assessments, and enrollments for eligible participants.
  • Ensure follow-up contacts for PrEP eligible participants for linkage to services and appointments.
  • Provide comprehensive ongoing PrEP navigation services for individuals vulnerable to HIV, including medication adherence counseling.
  • Develop and implement client-centered health education and risk reduction plans.
  • Conduct outreach and recruitment for PrEP services in community settings and health centers.
  • Document all client-related activities and referrals in the electronic medical record (EMR).
  • Assist in the development and training of peer educators for PrEP services.
