About The Position

Near North Health is seeking a Wellness West Medical Home Community Health Worker (MH CHW) to join our team. This role is part of an initiative on Chicago's west side that aims to improve the health of community members with chronic illnesses by addressing social determinants of health. The MH CHW will work closely with patients to enhance their self-management of diabetes, hypertension, and mild to moderate depression through education, assessment, and support in overcoming barriers to self-care. This position is integral to our mission of providing accessible and high-quality healthcare to all individuals, regardless of their financial circumstances.

Requirements

  • High school diploma or equivalent combination of education and experience.
  • Proven experience in a customer service role, preferably in healthcare or hospitality.
  • Excellent verbal and written communication skills.
  • Basic understanding of medical terminology and healthcare practices.
  • Proficiency in office software and electronic health records systems.

Nice To Haves

  • Knowledge of Chicago's West Side communities.
  • Experience working with underserved populations.
  • Previous experience in health education.
  • Familiarity with evidence-based psychosocial treatments for mental health disorders.
  • Experience working with patients who have diabetes, hypertension, and/or depression.

Responsibilities
