About The Position

The Wellness Specialist at WellLife Network is responsible for managing a comprehensive program of services that includes individual and group assessments, counseling, and linkage with various health, educational, entitlement, and vocational programs. The role focuses on ensuring successful and timely placement of consumers into the community while maintaining high standards of service delivery and compliance with regulations. This position involves direct interaction with consumers and requires a commitment to diversity, equity, inclusion, and belonging.

Requirements

  • High school diploma or GED required; Bachelor's in human services preferred.
  • Minimum of 2 years' experience working with individuals with mental illness required.
  • Two years experience working with Mental Health and Substance Abuse consumers preferred.
  • Experience working with behavioral health population preferred.
  • Computer proficiency, including email and electronic medical record documentation.
  • Valid New York State driver's license required.

  • Experience with case management software.
  • Knowledge of community resources for mental health and substance abuse services.

Responsibilities

  • Screen clients and conduct assessments to determine areas of need.
  • Coordinate case management services to ensure comprehensive development of an individual's service plan.
  • Facilitate obtaining entitlements for consumers.
  • Act as a clinical liaison.
  • Control the quality of services through regular supervision, case conferences, and consultations.
  • Establish and implement ongoing casework policies and procedures.
  • Develop treatment plans with ACT team members and consumers.
  • Manage timely collection and analysis of client and program data for reporting and evaluation.
  • Coordinate services with all members of the ACT Team.
  • Facilitate groups for consumers.
  • Ensure timely completion of documentation in compliance with agency and OMH regulations.
  • Maintain compliance of the physical plant with agency and OMH regulations.
  • Participate in OMH approved ACT Orientations and Trainings.
  • Operate an agency vehicle as needed.
  • Attend scheduled trainings relevant to the position.
  • Assist clients with case management needs, including transportation and benefits applications.
  • Maintain confidentiality at all times.
  • Represent the Program at meetings and trainings as needed.
  • Report and document all incidents to the supervisor within 24 hours.
  • Assist in planning, developing, and implementing goals and training for participants.
  • Act as a role model for participants in researching community resources.
  • Provide crisis intervention and mental health counseling as needed.
  • Input all visits and data into Vellohealth software application.
