About The Position

The Wellness Specialist at WellLife Network is responsible for managing a comprehensive program of services aimed at supporting individuals with mental health challenges. This role involves conducting assessments, coordinating case management services, and facilitating access to various health and educational resources. The specialist will work closely with the ACT team to ensure high-quality service delivery and compliance with regulations, while also providing crisis intervention and mental health counseling as needed.

Requirements

  • High school diploma or GED required; Bachelor's in human services preferred.
  • Minimum of 2 years' experience working with individuals with mental illness required.
  • Two years of experience working with Mental Health and Substance Abuse consumers preferred.
  • Experience working with behavioral health populations preferred.
  • Computer proficiency, including email and electronic medical record documentation.
  • Valid New York State driver's license required.

  • Experience with crisis intervention and mental health counseling.
  • Familiarity with community resources for mental health and substance abuse.

Responsibilities

  • Screen clients and conduct assessments to determine areas of need.
  • Coordinate case management services to ensure comprehensive development of individual service plans.
  • Facilitate obtaining entitlements for consumers.
  • Act as a clinical liaison among team members and consumers.
  • Control the quality of services through regular supervision and case consultations.
  • Establish and implement ongoing casework policies and procedures.
  • Develop treatment plans in collaboration with the ACT team and consumers.
  • Manage the collection and analysis of client and program data for reporting and evaluation.
  • Coordinate services with all members of the ACT team.
  • Facilitate group sessions for consumers.
  • Ensure timely completion of documentation in compliance with agency and OMH regulations.
  • Maintain compliance of the physical plant with agency and OMH regulations.
  • Participate in OMH approved ACT orientations and trainings.
  • Operate an agency vehicle as needed.
  • Attend scheduled trainings relevant to the position.
  • Assist clients with case management needs, including transportation and benefit applications.
  • Maintain confidentiality at all times.
  • Represent the program at meetings and trainings as required.
  • Report and document all incidents to the supervisor within 24 hours.
  • Assist in planning and implementing goals and training for participants.
  • Provide crisis intervention and mental health counseling as needed.
