What You’ll Do Performs Annual Wellness Visits, develops preventive care plans, and documents per CMS and provider guidelines. Manages care for high-risk and ACO patients, coordinating services and participating in daily/multidisciplinary huddles. Provides direct nursing care including triage, treatments, injections, labs, and patient admissions per protocol. Maintains compliance with licensure, safety, infection control, and quality improvement standards. Collaborates across departments, supports organizational mission, and upholds CommWell Health’s core values.

Requirements

• Required BLS Certificate from American Red Cross or American Heart Association
• Registered Nurse (RN) with current NC license
• Minimum One (1) year experience as RN, LPN, or other related medical field
• Clean driving record with current NC driver’s license and approved by corporate vehicle insurance vendor for driving privileges

Nice To Haves

• Prefer experience with Care Management, Value Based Care, Population Health, Medicare Annual Wellness.
