About The Position

Carelon Behavioral Health, a member of the Elevance Health family of companies, offers clinical mental health and substance use disorder management, employee assistance programs, work/life support, specialty programs for autism and depression, and insightful analytics. The Wellness & Recovery Specialist is responsible for care coordination and/or care management activities focused on the wellness and recovery of members. This is a field-based role, requiring associates to primarily operate in the field, traveling to client sites or designated locations as needed, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

Requirements

  • AA/AS and minimum of 2 years of experience in health services or behavioral health field or in a peer support services role; or any combination of education and experience, which would provide an equivalent background.
  • Valid, active, certified recovery specialist/coach certification and/or peer/family support specialist certification required.
  • Candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law.
  • New candidates in certain patient/member-facing roles are required to become vaccinated against COVID-19 and Influenza.

Nice To Haves

  • Knowledge of care-coordination and case management concepts strongly preferred.
  • BA/BS preferred.

Responsibilities

  • Identifies opportunities for engagement of members and their families in forming a supportive, recovery network.
  • Develops and implements provision of onsite psychiatric discharge planning education at Recovery and Resiliency sites.
  • Collaborates with Stabilization Teams as a member advocate in discharge planning education, resolution of barriers, and service transitions.
  • Acts as a resource for staff on decision making and problem solving.
  • Initiates and maintains contact with assigned individuals and providers to determine member’s response to services.
