Requirements

  • 2 Year/Associate Degree In Nursing, required or Vocational/Technical Diploma Professional Nursing Program, required.
  • 4 Year/Bachelor's Degree in Nursing, preferred.
  • Minimum 3 years of relevant work experiences as an RN with experience working in a multi-disciplinary team, required.
  • Current NYS licensure as a Registered Professional Nurse, required.
  • BLS (Basic Life Support) Certification, required.

Nice To Haves

  • Prior experience working with EPIC or other electronic medical record, preferred.

Responsibilities

  • Responsible for a full range of services related to promotion of preventive and wellness plans, illness prevention, and chronic condition management of the patient population.
  • Complete screenings, comprehensive nursing assessments, close care gaps, and provide extensive patient education.
  • Perform the Medicare Annual Wellness Visit for Medicare Part B beneficiaries under the direct supervision of an onsite physician.
  • Develop an individualized plan of care with the patient focused on prevention and wellness goals.
  • Identify patient risk factors and collaborate with or refer to primary care clinicians and other practitioners.
  • Coordinate outreach and scheduling activities including verifying eligibility for Medicare Annual Wellness services.
  • Conduct Pre-Visit Planning activities to plan for efficient/effective patient visit.
  • Perform all aspects of Medicare Annual Wellness visits including screenings, assessments and plan of care.
  • Promote wellness and illness prevention using clinical nursing knowledge.
  • Educate patients regarding lifestyle modifications, wellness promotion, illness prevention and chronic disease management.
  • Develop written, individualized prevention plan for each patient.
  • Educate patients regarding advance care planning and additional community resources.
  • Collaborate with physicians and other care team members to ensure patient receives referrals and follow up care.
  • Ensure patients are scheduled for recommended services with input from PCP.
  • Maintain relevant nursing knowledge of evidence-based strategies related to health promotion.
  • Ensure proper documentation in the EHR including closing data and care gaps.
  • Perform nursing functions including vital signs, immunizations, and collecting laboratory specimens.
  • Participate in the planning, validation, and monitoring of regional KPI's related to Medicare Annual Wellness Visit activities.
  • Maintain professional relationships with all internal and external colleagues.
  • Maintain confidentiality.
  • Perform other duties assigned.
