About The Position

The Wellness Nurse at The Phoenix at Lake Lanier is responsible for monitoring the health and wellbeing of residents, providing leadership support, and assisting with resident care and services. This role involves medication administration, communication with support services and families, and maintaining clinical quality assurance while promoting wellness as part of the Phoenix programs and services.

Requirements

• Must be a Licensed Practical Nurse or Registered Nurse.
• A minimum of one year of experience working with elders or disabled individuals, preferred.
• Demonstrated leadership capabilities.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
• Ability to work weekends, evenings, and flexible hours.

Nice To Haves

• Understanding of infection control procedures.
• Proficient in using MS Office.
• Understanding of advanced directives and end-of-life care.
• Knowledge of state regulations and compliance tools.

Responsibilities

• Identify ongoing needs and services of residents through assessment and Personalized Service Plan (PSP) process.
• Ensure proper follow-through and documentation for residents with changes in clinical care needs.
• Assist the Wellness Director with completing clinical sections of monthly wellness visits and PSP.
• Inform the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
• Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
• Provide direct resident care as needed.
