About The Position

The Wellness Navigator plays a key role within SCMHCC’s Certified Community Behavioral Health Clinic (CCBHC) model by supporting whole-person, integrated care. This position focuses on promoting wellness, recovery, and health equity through required screenings, care coordination, health education, and connection to community resources. The Wellness Navigator maintains an active caseload and works collaboratively with clients, families, internal treatment teams, primary care providers, and community partners to address behavioral health, physical health, and social determinants of health. Through client-centered engagement and advocacy, the Wellness Navigator supports access to care, continuity of services, and improved health outcomes.

Requirements

  • Bachelor’s level or equivalent experience required.
  • Ability to effectively relate to employees and the public in a positive way.
  • Ability to work effectively independently, or in a team with people of all socio-economic classes.
  • Produce documents written with clearly organized thoughts using proper sentence construction, punctuation and grammar.
  • Have excellent judgement, communication, and conflict resolution skills.
  • Have excellent ability to deal with diverse personalities, in a tactful, flexible and mature way.
  • Have a working knowledge of center programs, current policies and regulations governing
  • SCMHCC operations.
  • Individuals must be physically capable of operating the vehicle safely, possess a valid driver’s license and have an acceptable driving record.

Nice To Haves

  • Community Health Worker (CHW) certification is preferred.
  • Candidates who are not currently certified must demonstrate a willingness and ability to pursue Kansas Community Health Worker certification within a timeframe determined by SCMHCC.

Responsibilities

  • Screening and Assessment (CCBHC Required Services)
  • Conduct required wellness, behavioral health, physical health, substance use, and social needs screenings in accordance with CCBHC standards for children, adolescents, and adults.
  • Care Coordination and Follow-Up
  • Maintain an assigned caseload of clients receiving wellness navigation services.
  • Provide ongoing follow-up to support continuity of care, including phone outreach, rescreening, appointment coordination, and documentation of outcomes.
  • Coordinate care across internal and external partners.
  • Assist clients in navigating eligibility criteria, applications, and appeal processes for medical, behavioral health, social, educational, and supportive services.
  • Perform duties consistent with Community Health Worker core functions, including system navigation, care coordination, health education, and client advocacy to reduce barriers to care.
  • Documentation and Data Reporting
  • Accurately and timely document all screenings, client interactions, referrals, follow-ups, and outcomes in the electronic health record (EHR).
  • Ensure documentation meets CCBHC, organizational, regulatory, and payer requirements.
  • Assist with tracking referrals, external services, and engagement outcomes to support quality improvement and reporting needs.
  • Client Engagement and Education
  • Provide face-to-face, telephonic, and administrative support to clients and families.
  • Educate clients on preventive health, wellness strategies, and available services to support recovery.
  • Encourage client engagement in treatment, wellness activities, and community resources.
  • Support clients in identifying and overcoming barriers related to health, wellness, and recovery.
  • Community and Team Collaboration
  • Develop and maintain professional, collaborative relationships with SCMHCC staff and community partners.
  • Maintain current knowledge of community resources, services, and supports relevant to client populations.
  • Share information on community activities and resources with clients and staff to strengthen natural and informal support systems.
  • Participate in team meetings, trainings, and quality improvement initiatives as assigned.
  • Regular and predicable attendance and punctuality.
