Promotes wellness and disease prevention through education, special programs, and assessments.
Works with patients to develop self-help plans and ensures that patients adhere to their established goals
Coaches patients through individualized health plans by continuously encouraging healthy habits
Reports on patient health status and progress
Conducts wellness training courses
Ensures that wellness programs meet customer needs and satisfaction
Work may be periodically reviewed for accuracy and adequacy
Work assignments are varied and require interpretation and independent decisions on course of action
Majority of contact is within own function, area, or department and may be customer service oriented
