About The Position
Promotes wellness and disease prevention through education, special programs, and assessments.
Requirements
- Bachelor’s Degree or equivalent combination of education and related experience
- 1-3 years of technical experience
Responsibilities
- Works with patients to develop self-help plans and ensures that patients adhere to their established goals
- Coaches patients through individualized health plans by continuously encouraging healthy habits
- Reports on patient health status and progress
- Conducts wellness training courses
- Ensures that wellness programs meet customer needs and satisfaction
- Work may be periodically reviewed for accuracy and adequacy
- Work assignments are varied and require interpretation and independent decisions on course of action
- Majority of contact is within own function, area, or department and may be customer service oriented
