About The Position
The Wellness Coordinator role at Optum is a non-clinical position focused on providing care coordination and administrative support to members with behavioral health and substance abuse needs. The coordinator acts as a primary care navigator, engaging members, screening for care gaps, addressing social determinants of health (SDOH), and developing individual care plans. This position also involves administrative tasks to support the Behavioral Health Case Management department.
