About The Position

AdvisaCare is seeking an experienced, motivated, and reliable Wellness Coordinator to work at one of their Independent Living Communities in Fort Gratiot, MI. The Wellness Coordinator will have the unique opportunity to grow their own business within AdvisaCare, much like a franchisee. This role involves assessing the needs of clients in the community setting and admitting them to services, ensuring that AdvisaCare's standards of care are provided by Home Health Aides (HHA) and Certified Nursing Assistants (CNA), and building personal relationships with clients. The Wellness Coordinator is responsible for the entirety of their community, which includes providing training and ongoing support to staff, collaborating with the Executive Director, and performing light office duties. A willingness to provide hands-on care is essential. To perform this job successfully, the individual must be able to perform each essential duty satisfactorily and possess the physical ability to perform job-related duties such as lifting, standing, bending, stooping, stretching, walking, pushing, and pulling.

Requirements

  • Minimum one (1) year experience
  • Updated Tuberculin (TB) test and current CPR certification
  • Clear Criminal History/Drug Screen
  • Critical thinking, task competence and the ability to think on your feet
  • Problem solving skills and the ability to multi-task
  • Physical ability to perform job related duties which may require lifting, standing, bending, stooping, stretching, walking, pushing and pulling
  • Willingness to do Hands on Care

Responsibilities

  • Hiring/Training/Supervising Your Staff - Team Lead, Med Techs, Home Health Aides
  • Building Relationships with Staff, Residents, Families and Community Directors
  • Assessments of new residents or those being discharged back from a hospital/rehab center
  • Acts as a liaison and maintains contact with clients, therapist(s), Regional Care Manager, family and other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals.
  • Prepares appropriate medical documentation on all clients, including any case conferences, client contacts, medication order changes, re-certifications, progress updates and care plan changes.
  • Prepares visit/shift reports, updates/summarizes client records and confers with other health care disciplines in providing optimum client care
  • Attend the functions necessary to spread our “gospel” (this may occasionally mean early and late work hours).
  • Confers with the client, physician, and family in developing the initial plan of treatment based on client needs and initial patient assessment.
  • Provides hands on care, management and evaluation of the care plan and teaching of the client under the Regional Care Managers supervision.
  • Coordinates appropriate care encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
  • On-Call after hours/weekends
  • Oversight for all shifts
  • Working the floor as support is required
