Through structured needs assessments, direct referrals, and ongoing case support, this position addresses social determinants of health that impact workforce stability, engagement, and retention.

Requirements

  • Bachelor’s degree required (Health Education, Social Work, Public Health, or related field preferred).
  • Minimum two years of experience in healthcare, case management, community health, or social services preferred.
  • Strong verbal and written communication skills.
  • Ability to maintain confidentiality and manage sensitive situations with discretion.
  • Ability to work onsite across multiple campuses and support all shifts, including occasional evenings and weekends.
  • Ability to manage competing priorities and work effectively under time constraints.

Nice To Haves

  • MSW or Certified Health Coach preferred.

Responsibilities

  • Conduct comprehensive needs assessments to identify barriers related to housing, food access, transportation, legal services, financial hardship, and mental health care.
  • Analyze assessment data to identify trends and inform proactive support strategies.
  • Utilize the Prisma Health LiveWell platform to connect team members with internal and community resources.
  • Provide hands-on assistance with benefit enrollment, emergency aid applications, and resource navigation.
  • Serve as liaison to the Emergency Care Fund, coordinating documentation and approvals to ensure timely financial assistance.
  • Maintain structured follow-up meetings at 30/60/90 days and one year to monitor stability and measure program effectiveness.
  • Partner with HR and department leaders to ensure safe, confidential, and appropriate workplace support.
  • Deliver educational sessions on whole-person health, financial wellness, and available community resources.
  • Maintain accurate documentation and reporting to track utilization, outcomes, and program impact.
  • Build and maintain relationships with local nonprofits, housing authorities, food banks, legal aid providers, behavioral health agencies, and faith-based organizations.
  • Represent Morrison Healthcare in community coalitions and outreach initiatives focused on workforce well-being.
  • Participate in resource fairs and community events to strengthen partnerships and expand access to services.
  • Collaborate with community partners to enhance resource availability and support long-term workforce stability.
