The Wellness Coordinator at UnitedHealth Group plays a crucial role in providing non-clinical care coordination and administrative support to members with behavioral health and substance abuse needs. This position involves engaging with members, identifying gaps in care, and developing individual care plans to enhance access to necessary services. The role also includes administrative tasks to support the Behavioral Health Case Management department, ensuring effective communication and resource allocation.

  • Work in an inbound member queue to assist with care coordination.
  • Document interactions in multiple electronic documentation systems.
  • Coordinate information with clinical team partners.
  • Provide education on behavioral health benefits to members.
  • Contact members to screen for and identify gaps in care.
  • Provide network referrals and community resources for mental health and substance use treatment.
  • Offer administrative support to the Behavioral Health Case Management department.
  • High School Diploma/GED
  • Unencumbered Missouri State driver's license
  • Minimum automobile liability insurance as required by Missouri law
  • 2+ years of managed care or call center experience
  • Familiarity with working on computers
  • Ability to travel 25% in the assigned area with reliable transportation.
  • 3+ years of experience in a Medical/Behavioral health setting
  • Casework experience at hospitals or community mental health centers
  • Dual diagnosis experience with mental health and substance abuse
  • Experience coordinating benefits and utilizing multiple resources for patients
  • Knowledge of community-based resources and services
  • Creative problem-solving skills to meet members' needs
  • Initiative in achieving individual and team goals
  • Flexibility and attention to detail
  • Solid oral and written communication skills
  • Effective organizational and scheduling skills
  • Experience handling sensitive issues confidentially
