About The Position

The Wellness Coordinator plays a pivotal role in delivering integrated, value-based care to patients enrolled in wellness programs at Your Health . This position ensures seamless coordination between the wellness provider, clinic team, centralized medication ordering, and billing departments. The ideal candidate will be an LPN or RN with strong clinical, administrative, and case management experience. The Wellness Coordinator is responsible for medication compliance tracking, patient engagement, and ensuring all aspects of wellness services are clinically appropriate, financially accounted for, and aligned with broader primary care goals.

Requirements

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) required; active state licensure in good standing.
  • Minimum 2+ years of clinical experience , preferably in a wellness, primary care, or case management setting.
  • Experience with EHR systems and clinical documentation (AthenaHealth experience a plus).
  • Knowledge of value-based care , chronic care management, and wellness-focused interventions preferred.
  • Strong organizational, communication, and interpersonal skills.
  • Detail-oriented with a commitment to compliance, care quality, and patient advocacy.

Nice To Haves

  • Working knowledge of HEDIS, Star Ratings, and risk adjustment.
  • Experience with weight management, hormone replacement therapy, or supplement-based programs.
  • Ability to manage multiple priorities while maintaining accurate clinical and financial records.
  • Collaborative mindset with a patient-first orientation and a proactive problem-solving approach.

Responsibilities

  • Serve as the primary point of contact for patients enrolled in wellness programs, ensuring timely follow-up and coordination of services with primary care providers and specialists.
  • Provide direct patient care support, including administering injections , assisting with pellet procedures , and delivering in-clinic wellness therapies .
  • Perform ongoing case management to monitor clinical outcomes, ensure follow-up appointments are completed, and help patients navigate their wellness, chronic disease, and preventative care needs.
  • Collaborate with the central medication ordering staff to ensure all medications/products are ordered timely , tracked, and documented accurately.
  • Log and reconcile dispensed medications/products in coordination with the billing and compliance teams , maintaining alignment with regulatory and payer requirements.
  • Monitor compliance metrics and work closely with the care team to ensure documentation supports value-based care initiatives , including HEDIS measures and risk adjustment efforts.
  • Ensure payments for wellness services (e.g., peptides, hormone therapy, weight loss programs) are collected, documented, and logged into the EHR or billing systems accurately.
  • Partner with billing team to resolve payment discrepancies , ensure proper coding, and verify that products/services are linked to appropriate charges .
  • Work the RPM referral bucket specific to wellness patients, ensuring appropriate enrollment, education, and onboarding into RPM and remote care programs .
  • Monitor data within Athena and PBI and flag clinical trends that require intervention or escalation to the wellness provider or PCP.
  • Support patient engagement through proactive outreach, education, and motivation for adherence to personalized wellness and preventative care plans.
  • Collaborate with the clinics wellness provider and clinical team to enhance patient outcomes, satisfaction, and retention.
