About The Position

We are looking for a Medical Case Manager Supervisor to join our team at our Emerald clinic. Purpose The Wellness Case Manager (WCM) is an important team member supporting the Wellness Center and Emerald Clinic, which is essential for achieving the dual mission of Full Circle Health. The WCM shall be accountable for 1) Understanding and complying with the Ryan White Part B and C Grant expectations and requirements, 2) Providing case management services as part of a multidisciplinary team to eligible patients accessing services at the Wellness Center and 3) Maintain an understanding of HIV treatment and care guidelines. The WCM works closely with Wellness Center Staff, as well as team members from Emerald nursing and reception, pharmacy, and providers. Requirements

  • Degree in Social Sciences, Nursing, Social Work, Psychology or related field from an accredited four-year college or university, preferred.
  • 3 or more years working in a social service organization or healthcare setting providing service delivery.
  • 2 or more years working in a supervisory role.
  • Chronic Care Management Certificate, required within 4 months of hire.
  • Excellent verbal and written communication skills
  • Our organization is a tobacco-free workplace.
  • We require immunizations for all staff to include an annual influenza vaccine as a requisite for employment. Newly hired staff who are not able to be vaccinated due to medical reasons or sincerely held religious beliefs must apply for vaccination exemption prior to employment
