Wellness and Recovery Specialist Location: Virtual: This role enables associate to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. The Wellness and Recovery Specialist is responsible for care coordination and/or care management activities focused on the Wellness and Recovery of members. How you will make an impact: Responsible for coordinating and managing care activities with a focus on the wellness and recovery of members. Key responsibilities include identifying opportunities to engage members and their families in forming a supportive recovery network. Developing and implementing onsite psychiatric discharge planning education at Recovery and Resiliency sites. The role collaborates with Stabilization Teams, advocating for members in discharge planning education, resolving barriers, and facilitating service transitions. Serves as a resource for staff with decision-making and problem-solving. Initiating and maintaining contact with assigned individuals and providers to assess members' responses to services.
