About The Position

The WelbeHealth PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. It’s our mission to serve the most vulnerable seniors with better quality and compassion in a value-based model. The Welbe Advocate Nurse supports the WelbeHealth team’s ability to provide outstanding continuity of care when the PACE center is closed within their scope of practice. The Welbe Advocate Nurse is accountable for answering phones after hours (evening, nights, weekends, and holidays), coordinating logistical issues that arise both through phone contact and visits to the participant’s residence and/or acute care settings as necessary to promptly resolve issues, and consulting with other clinical staff on call, as needed. Requirements

  • Graduate of an Accredited School of Nursing and have an Unencumbered RN license
  • Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
  • Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
  • Ability to work independently with minimal supervision.

Responsibilities

  • Handle all calls/contacts (inbound/outbound) as assigned, responding as appropriate within their scope of practice, consulting with other clinicians, including on-call providers, when necessary.
  • Coordinate telehealth encounters between participants and clinicians as required.
  • Ensure timely care delivery.
  • Trouble-shoot and effectively resolve logistical care delivery issues such as transportation, medication delivery, and emergency room/hospital discharge as needed when the daytime care teams or responsible central teams are notavailable.
  • Trouble-shoot and resolve other basic issues; escalate to management as necessary.
  • Support the clinical care teams and home health staff to manage smooth care transitions between settings (hospitals, skilled nursing facilities, etc.), escalating changes in the participant’s condition as appropriate.
  • Able to work assigned shift which may include days, evenings, nights, weekends, holidays, and overtime.
  • Exercises flexibility in performing assignments as business needs evolve.
