Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies. Specifically, the Weight Management Primary Care Pharmacist will be a primary care pharmacist responsible for managing patients who fall out of a nurse protocol for weight management medications, primarily GLP-1 agonists. Responsibilities include Medication Therapy Management (MTM) or Medication Management Services (MMS) including comprehensive medication reviews, targeted medication reviews, and non-visit care.
