About The Position

CentraCare Willmar is seeking a Nurse Practitioner or Physician Assistant to join our team of Physicians and Providers working with our Weight Management Team. In this role, you will be a member of our collaborative team of Physicians, Providers, Registered Dietitians, and nurses working to provide patient and family centered care throughout the practice. Position Details - Options to work in St. Cloud and/or Willmar, MN clinics - Full Time, 1.0 FTE - Monday- Friday, 7:30am- 4:30pm. CentraCare's Weight Management Team is a progressive and comprehensive obesity medicine department that leads the state in our field. We want to treat, help, and have an impact on the whole person. Our team of physicians, advanced practice providers and dietitians is very collaborative and works well together.

Requirements

• Be certified as a Nurse Practitioner or have a current Minnesota Physician Assistant Licensure
• Have a current American Heart Association BLS Healthcare Provider card
• Have a Drug Enforcement Agency (DEA) registration number prior to receiving clinical privileges

Nice To Haves

• Have a minimum of 2 years of experience working independently with your own patient panel in one of the following areas: Family Practice, Weight Management, Endocrine, Chronic Disease Management

Responsibilities

• Obtain patient histories and perform physical examinations on patients
• Formulate the appropriate differential diagnosis based on history, physical examination, and clinical findings
• Order and evaluate pertinent laboratory data and diagnostic procedures
• Prescribe pharmacological agents and non-pharmacological therapies
• Plan a course of treatment with the patient
• Consult with physicians as needed and make appropriate referrals for care
