Position Summary Weekends Only, Every Saturday & Sunday. The Wound Care / Treatment Nurse is responsible for coordinating and providing comprehensive skin and wound care services to residents in accordance with federal and state regulations, facility policies, and standards of practice. This role ensures timely and thorough weekly skin assessments, treatment documentation, and communication of resident skin and wound status to the interdisciplinary team. The Wound Care Nurse also assists in educating staff on skin integrity practices and participates in the facility’s wound prevention and management program, while maintaining readiness to perform general nursing duties as needed. Essential Duties and Responsibilities Clinical Duties: Complete and document weekly skin assessments for all residents, identifying changes in condition, areas of redness, pressure, or skin breakdown. Perform and document weekly wound observations for all residents with open wounds, pressure injuries, or other skin impairments. Accurately stage, measure, photograph, and describe all wounds per policy and best practice. Develop, implement, and update wound care treatment plans in collaboration with the physician, DON, and interdisciplinary team. Administer treatments and dressings as ordered; monitor residents for signs of infection or delayed healing. Ensure all skin and wound care documentation is complete, accurate, and compliant with regulatory standards. Coordinate with dietary, therapy, and nursing staff to promote wound healing and prevention through appropriate interventions. Monitor and assist with pressure injury prevention measures, including turning/repositioning programs and support surfaces. Track and trend skin/wound data and participate in QAPI and wound rounds as assigned. Education & Collaboration: Provide education to nursing staff on proper wound care techniques, infection prevention, and skin integrity preservation. Collaborate with the medical director, attending physicians, and consultants (e.g., wound care specialists) as needed. Participate in interdisciplinary meetings and care plan reviews to ensure appropriate care coordination. General Nursing Home Duties: Provide direct resident care and medication administration as needed. Respond to emergencies promptly and effectively. Support and model facility policies, infection control practices, and resident rights. Participate in surveys, audits, and other facility inspections as directed. Qualifications Current, active RN or LVN/LPN license in good standing with the state board of nursing. Minimum of 1 year of nursing experience in long-term care, skilled nursing, or similar healthcare setting preferred. Prior wound care certification (WCC, WOCN, or equivalent) preferred but not required. Knowledge of wound care principles, infection control, and documentation standards. Excellent communication, critical thinking, and organizational skills. Ability to work independently and as part of an interdisciplinary team. Commitment to resident-centered care and quality improvement. Physical Requirements Ability to stand, walk, bend, and lift up to 50 lbs. Visual and auditory acuity sufficient to assess and monitor residents’ conditions.
