Coordinates new employee orientation, manages all residents care needs monitors residents conditions, assists with admissions/discharges and communication to resident family members, as well as, supports, evaluates and develops staff.

Requirements

  • RN or LPN from an accredited or state approved school of nursing with current license to practice in the state of Indiana.
  • Two years experience in a long-term care setting with charge or comparable responsibilities.
  • Have a genuine interest in working with the geriatric population.
  • Have the ability to motivate, supervise and delegate.
  • Self-motivated and able to prioritize tasks, effectively manage time and communicate with key players.

Responsibilities

  • Supervises direct care of residents and nursing personnel on his/her shift.
  • Administers direct care as necessary.
  • Keeps physicians and families aware of changes in residents’ condition.
  • Address any immediate concerns, complaints or issues with resident care.
  • Supervises all nursing personnel working on weekend shifts.
  • Makes daily rounds at least 2 times to assess resident conditions and needs.
  • Observe direct care administered by staff to provide suggestions for quality assurance and performance improvement.
  • Exhibits competence with and able to train others on the proper use of all resident care equipment, such as lifts, VSMs, etc.
  • Oversee and assist with completion of all medical record documentation.
  • Maintain clean, odor-free, orderly, comfortable and safe environment for the residents and employees.
  • Comply with and monitor adherence to infection control and medication policies and procedures.
  • Review and/or complete accident and incident reports and medication error reports according to facility policy.
  • Use, interpret and convey information related to resident care policies and procedures.
  • Give written report to the DON/IDT on a daily basis of all pertinent care issues.
  • Report defective equipment of potential hazards for repair or immediate remediation.
  • Maintain a courteous and helpful attitude towards residents, visitors and staff members.
  • Communicate resident concerns with other disciplines as necessary.
  • Maintains the self-respect, personal dignity, and physical safety of each resident.
  • Encourages restorative care in order to obtain maximum resident independence.
  • Assist with quality control and quality assurance data gathering.
  • Identify specific learning needs of the unit and implement education for staff.
  • Participate in the orientation and evaluation process of all unit employees assigned to weekend shifts.
  • Implement disciplinary action on employees not following facility procedures and policies.
  • Assure completeness and thoroughness of orientation for new employees.
  • Manages communication sheets to nursing units daily on weekend shifts.
  • Immediately notify UM, DON and Administrator of any unusual occurrence that may be reportable to the Indiana Department of Health.
