About The Position

The Weekend RN Clinical Liaison functions as a facilitator for hospitals and community services in need of referring patients to Longleaf Hospice and Palliative Care, assisting in the clinical assessment of patients to ensure they are appropriate for admission, while also serving as a business development professional who focuses on appropriate referrals by creating and sustaining business partnerships with facilities and hospitals.

Requirements

  • Graduate of an approved school of professional nursing and currently licensed as a Registered Nurse in the state(s) in which practicing
  • CPR certification
  • Experience in health care industry with weighted emphasis in sales/marketing
  • Must have proven interpersonal, leadership, customer service and team building skills and ability to converse with Physicians and Senior Managers and an understanding of financial management
  • Proficiency using Microsoft Word, Excel, and PowerPoint
  • Demonstrated ability to prioritize and manage time effectively and make independent decisions when necessary is essential
  • Dependable transportation, valid driver’s license, good driving record and auto insurance

Responsibilities

  • Establishes and maintains significant professional relationships, working closely with community, hospital, and facility education representatives to meet growth and development targets and maintain market relationships
  • Explains hospice services and benefits to patients, family members, etc. to facilitate an accurate perspective of hospice and palliative services
  • Provides education to hospitals and facilities on the purpose, process, and goal of general inpatient care
  • Facilitates the admission process to and from hospital and long-term care-based referral sources
  • Assesses referred patient’s clinical needs and disease status and confers with patient’s attending physician and/or hospice physician to establish eligibility
  • Facilitates patient discharge from hospital to home by establishing initial needs and coordinating transportation, durable medical equipment, medication needs, supplies, and admission; may perform hospice admissions and other visits as staffing needs required
  • Oversees GIP patient admission process based on identified symptom management needs for that level of care, working closely with facilities and hospitals to identify, transition and admit patients appropriately for general inpatient services
  • Ensures all forms are completed, dated, signed, and turned into the Intake Department and copies are provided to the patient/responsible party
  • Maintains accurate records, documents actions, maintains control of budgeted expenses and submits required reports on time
  • Fosters a positive attitude and builds morale among staff, promoting open communication with all departments
  • Participates in Company functions to build public awareness of our services
  • Continues professional growth through attendance at workshops, professional in-services, etc.
  • Maintains medical ethics in compliance with company policies and procedures
  • Maintains compliance with all state and federal regulations pertaining to the operations of a Medicare certified hospice agency
  • Performs other duties as assigned
