About The Position

The Registered Nurse Admissions within scope of practice, coordinates input of evaluations, assessments and/or care plan process data. Maintains compliance in accordance with current practice standards, physician’s orders, agency policies and procedures, and local, state and federal regulations. This position is also accountable for the delivery of professional nursing services, within the scope of the professional’s education and experience, in compliance with corporate standards, federal, and state laws. The work schedule includes Friday, Saturday, and Sunday (12 hours) with an additional 4 hours on Monday morning to finish paperwork. The nurse will be responsible for 3 admits each on Friday, Saturday, and Sunday, and will also be on call for the weekend.

Requirements

• Registered Nurse with a minimum of one year experience in home health or hospice nursing preferred.
• Ability to form relationships and maintain rapport with patients, families, and referral sources.
• Excellent communication skills, both verbal and written.
• Computer skills preferred.
• Ability to travel 50-75%.

Responsibilities

• Coordinate input of evaluations, assessments, and care plan process data.

• Deliver professional nursing services in compliance with corporate standards and laws.
• Complete paperwork on Monday mornings.
• Participate in monthly nurse meetings.
