Description The Hospice Admission Nurse is the first clinical impression for patients and families facing terminal illness and should convey comfort and assurance to all involved. For this role, you need to be a registered nurse (RN) who sees the value of hospice care in alleviating pain and suffering. A compassionate nature, knowledgeable in the caring for the terminally ill and excellent interpersonal skills are required to be successful in this role. The goal is to help patients live their final days with dignity and comfort. Capstone Hospice is currently in search of an Admissions Hospice Nurse to join our dedicated hospice admissions team. This position will require the admission nurse to process and complete admissions to our program. The Admissions Nurse should have the expectation of completing 1-2 admissions each day. The position requires driving to the patient's location to complete the admission within our service area. This position has the expectations of working part time every other weekend (Saturday & Sunday) 10:00am - 7:00pm. Candidate must be a Registered Nurse (RN) that can practice in the state of Georgia. Candidate should have strong hospice knowledge to be successful in this role. Go to our website to learn more about Capstone Hospice: www.capstonehospice.com Requirements Candidates must meet the following qualifications and experience to be considered: Graduate from an accredited school of nursing Currently licensed as a registered nurse to practice in the state of Georgia Minimum of 2 years as a registered nurse Active patient contact within the past 2 years, preferred 1-2 years hospice admissions knowledge or experience Active CPR certificate Must have current valid driver's license, auto liability insurance and your own reliable transportation. Must be able to provide evidence of annual TB test and other state required tests or examinations. To be successful in this role you should have the following knowledge & skills: Ability to work independently Make accurate judgements Strong nursing care processes and medication guidelines/side-effects Strong nursing assessment skills Proficient in the use of electronic medical record and hand-held tablet Proficient in English (understanding and articulation) Excellent communication skills Attention to detail Organized, efficient with strong time -management Acceptance of and adaptability to different social, racial, cultural and religious modes. Responsibilities: Complete admissions per day when needed Collaborate with physicians and other professionals to form and initial plan of care Complete documentation within 12-24 hours of admission visit Provide support and comfort according to individual patient's needs Work with caregivers and patients to assess goals of care Maintain accurate records Position Factors: Requires physical effort most of the day including, walking, exposure to humidity changes. Exposure to patient homes, including their families and pets within the home. Exposure to different social, racial, cultural and religious modes. Must possess sight/hearing senses or use of appropriate adaptive devices that will enable senses to function at a level to meet the essential duties of the position. Exposure to mental and emotional stress of the patients, caregivers and families. This is a part-time position eligible for Capstone Hospice 401k retirement plan, monthly cell phone allowance and mileage reimbursement.
