Requirements

Must be 18 years of age or older
Must have an active LPN license in the state of Indiana

Nice To Haves

At least one-year of related experience is preferred

Responsibilities

Provide direct care to residents by monitoring their condition, administering medications, and performing necessary tasks such as taking vital signs and documenting care
Collaborate with the interdisciplinary team to develop and implement care plans that enhance the physical, social, and mental well-being of residents
Ensure that all care is compliant with regulatory standards
Assist with staff training and participate in quality assurance activities
Maintain clear communication with residents, families, and medical staff about changes in care
