Weekends (Fridays, Saturdays, Sundays and Mondays) Intake Coordinator
About The Position
Visiting Nurses Association (VNA) is seeking a clinically knowledgeable Weekend Intake Coordinator provide coverage on Fridays, Saturdays, Sundays and Mondays with LPN licensure or related clinical experience to support our Home Health and Hospice programs. This role serves as a key clinical and administrative liaison during weekend operations, ensuring timely intake, appropriate service coordination, and continuity of patient care. The ideal candidate demonstrates strong clinical judgment, attention to detail, and the ability to assess referrals efficiently while maintaining strict confidentiality, dependable, professional and proper time management skills.
Requirements
- Active LPN license in the state of Kansas or equivalent clinical experience
- Experience in Home Health, Hospice, or clinical care setting preferred
- Strong understanding of clinical terminology and patient care workflows
- Excellent communication and critical-thinking skills
- Ability to work independently and manage time-sensitive referrals
- Reliable weekend availability required

Responsibilities
- Review and clinically assess Home Health and Hospice referrals for appropriateness and urgency
- Obtain and verify referral documentation, physician orders, and insurance information
- Collaborate with on-call nurses, case managers, and leadership to coordinate weekend admissions
- Enter accurate referral and patient data into the electronic medical record (EMR)
- Communicate professionally with referral sources, patients, families, and internal clinical staff
- Apply clinical knowledge to identify priority cases and escalate concerns appropriately
- Ensure compliance with HIPAA, CMS, and state regulations
- Maintain clear and timely documentation of all intake activities
