Presbyterian Homes & Services - Optage Hospice program is seeking an experienced Hospice RN passionate about providing quality end-of-life care to a diverse group of clients. This is a Part Time position which requires working every weekend (both Saturday and Sunday), with a rotating holiday schedule. The ability to work additional days (Friday/Monday) to increase hours up to FT is an option as desired. EEO/AA The Hospice Visit Nurse is an essential part of the care team that provides compassionate and dignified support to individuals and their families during the end-of-life process. This role involves adapting work routines to the clients' priorities while also supporting their personal preferences. The Hospice Visit Nurse works in a supportive role to the Hospice Interdisciplinary Group (IDG) team. The ideal candidate for this position should have knowledge of hospice philosophy, the hospice Medicare benefit, and the dying process, as well as experience in hospice case management. Flexibility is a top priority for the Hospice Visit Nurse, as this position involves a wide variety of nursing visits based on the individual needs of clients, mostly focused on end-of-life visit needs. The Hospice Visit Nurse will be required to travel throughout the metro area, so they must be comfortable with driving. This position also requires highly developed communication skills, including communicating with patients, family members, members of the IDG team, on-call nurses, and the supportive staff at the various housing choices where patients live.
