About The Position

The Hospice Weekend Team Manager will supervise and manage the hospice weekend colleagues for one or more of the HHCAH Hospice Regions. As part of a person-centered care model, this role will assist in management of daily operations of an interdisciplinary care team. This role will ensure the delivery of outcome based, cost effective care by utilizing metric-driven decisions. The primary purpose of this role is to oversee all patient care services and personnel for the Weekend program. This role will collaborate with all team members to achieve exceptional outcomes. Assures the development of personnel qualifications and policies. Identifies and supports the professional development needs and competencies of weekend staff through home visits that allow for supervision and evaluation of clinical performance. Develops and follows through individualized development plans. Reviews and analyzes team and patient metrics to ensure alignment with regional hospice initiatives. Provides staff guidance in making clinical and case management decisions focused on clinical excellence for end-of-life care and symptom management. Directs staff in the Medicare Hospice Benefit guidelines. Accountable for weekend team performance in achieving desired clinical and operational performance measures; assures that clinical and functional outcomes for assigned hospice region(s) are achieved at the highest level possible. Maintains compliance with regulatory agency policies and procedures by providing staff guidance in making clinical and case management decisions that are cost effective and focused on clinical outcomes. Determines staffing needs and coordinates hiring to meet service demands for all HHCAH geographic areas. Utilizes clinical software to identify opportunities for education, most appropriate utilization of services, oversee clinical progression of patients and trend team outcomes and performance. Resolves patient care problems collaboratively with internal/external customers – collaborates with peers throughout the Hospice regions. Oversees the daily patient assignments for weekend staff based on client needs and staff competency to maintain team productivity standard. Ensures that patient needs are continually assessed though the following: Ensures that an individualized plan of care is developed, implemented and updated as needed for all patients. Ensures coordination of patient care, including communication with all physicians involved in the plan of care. Hires, trains, coaches and evaluates staff. This role is also responsible for completion of personnel evaluations according to region timelines. Participates in preceptor programs and student education regarding hospice and palliative care end of life issues.

Requirements

  • Bachelor's Degree in Nursing
  • Home-based hospice care experience required
  • Current discipline licensure to practice in the State of Connecticut
  • Able to coordinate and manage patient care needs recognizing issues that require intervention
  • Able to communicate effectively with patient, family, and team members to reach optimum patient outcomes
  • Ability to lead and make sound decisions
  • Proven ability to work effectively independently and in a team environment
  • Must be comfortable and flexible with ambiguity and change processes
  • Excellent interpersonal skills
  • Demonstrates competence, including knowledge and behaviors, to meet age specific patients’ needs
  • Maintains infection control policies related to patient care
  • Computer literacy and ability to navigate through computer systems

Nice To Haves

  • Master's Degree preferred
  • Five+ years social work or nursing experience within a hospice agency or related community health program which included care of the sick at home.
  • Hospice experience and case management certification preferred
