Requirements

• VT Licensed RN (BSN preferred)
• Home care experience and/or previous case management experience in a home care setting and discharge planning and/or utilization review experience.
• Valid driver’s license

Responsibilities

• Deliver high quality care directly to individuals within the comfort of their homes in a designated geographic territory.
• Collaborate with colleagues, local physicians, and community providers to provide exceptional health care to our clients.
