Weekend Start‑of‑Care (SOC) Admissions RN – Home Health (PRN / PPV) Puget Sound Home Health and Hospice – Medicare 5‑Star Agency Puget Sound Home Health and Hospice is committed to exceptional care, ethical practice, and our Core Values of CAPLICO. We are seeking a Weekend SOC Admissions RN to support our growing Home Health team in Pierce and King County! Requirements

  • Current WA RN license
  • 1+ year clinical nursing experience in a Home Health agency
  • Current AHA BLS
  • Strong assessment and independent practice skills
  • Reliable transportation

Responsibilities

  • Complete weekend Start‑of‑Care admissions
  • Perform comprehensive assessments and OASIS documentation
  • Establish the initial Plan of Care
  • Educate patients and families
  • Coordinate with physicians and the interdisciplinary team
