Are you living your life's work? The Clinical Call Center RN supports clinical operations by processing and addressing telephone calls from patients, caregivers, staff, and physicians. The Clinical Call Center RN will triage clinical concerns and questions from the patient and caregiver, perform remote assessments via phone and/or video, and initiate clinical responses holistically and according to best practice. Additionally, this position will provide support to field clinicians including advising and clinical problem solving. The Clinical Call Center RN will assist field clinicians with after visit follow-up tasks and activities including communication with referring/prescribing providers associated with the patient's case.

Requirements

  • Bachelor of Science in Nursing (Required)
  • 2+ years related experience in home health or inpatient care.

Responsibilities

  • Assists with review of telehealth data and responds according to established protocols. Follow-up with physician, as necessary. Assures clinical information, responses, and actions are documented in the medical record.
  • Exhibits in depth clinical and operational knowledge across PMAH agencies including all home health and infusion services.
  • Responds to incoming calls from patients, caregivers, staff, and physicians. Facilitates actions to coordinate patient care using established clinical algorithms, protocols, and best care practices.
  • Seeks direction from, and collaborates with, home health managers and directors appropriately.
  • Documents calls, care coordination, and physician orders promptly in accordance with agency requirements.
  • Facilitates communication with physician practices and all field staff through verbal and written means.
  • Closely collaborates with hospital providers, case management, home health clinical field staff, and home health leadership.
  • Advocates for patients and ensures safe transitions in care by obtaining appropriate physician orders and procurement of DME and other supplies.
  • Advises referral sources, patients, and caregivers providing information and access to the full complement of services and specialty programs throughout PMAH.
  • Assists with quality audits and program development in collaboration with home health directors.
  • Develops and maintains professional demeanor and rapport with hospital staff, physicians, patients, caregivers, agency staff, and other referral sources. Utilizes effective communication skills in all interactions.
  • Manages multiple priorities across multiple teams and agencies, responding to and prioritizing requests to follow up with patients.
  • Demonstrates compassion and knowledge in all interactions with patients, family, and caregivers, as patients transition onto and receive PMAH services.
  • Implement service recovery solutions as appropriate.
