About The Position
Weekend Baylor Registered Nurse (RN) – Home Health Make a real impact with a schedule that works for you. Our Weekend Baylor RNs provide one-on-one skilled nursing care in the home, helping patients maintain independence while receiving high-quality, personalized care. This role offers autonomy, strong team support, and meaningful patient relationships. Schedule (Full-Time, Exempt) Saturday & Sunday: 7:00 AM – 8:00 PM Monday: 4 hours for follow-ups and documentation
Requirements
- Active RN license
- BSN + 1 year experience OR ASN/Diploma + 2 years experience
- Current CPR certification
- Valid driver’s license and reliable transportation
Nice To Haves
- Home health experience
- Behavioral or mental health experience
Responsibilities
- Perform patient assessments and develop individualized care plans
- Deliver skilled nursing care, including medication and treatment management
- Monitor patient progress and adjust care as needed
- Educate patients and caregivers on health management and self-care
- Coordinate with physicians and interdisciplinary teams
- Supervise Home Health Aides and LPNs
- Complete timely and accurate clinical documentation (including OASIS)
