About The Position

This position is for a Weekend Behavioral Health Nurse who will deliver meaningful, patient-centered care in the patient's environment. Key responsibilities include performing patient assessments, developing individualized care plans, and providing skilled nursing care such as medication and treatment management. The nurse will monitor patient progress, adjust care as needed, and educate patients and caregivers on health management. Coordination with physicians and interdisciplinary teams is essential, as is the supervision of Home Health Aides and LPNs. The role also requires timely and accurate clinical documentation, including OASIS.

Requirements

• Active RN license
• BSN + 1 year experience OR ASN/Diploma + 2 years experience
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities
