About The Position

The Weekend Baylor Home Health Registered Nurse is responsible for providing comprehensive nursing assessments, planning, and care to optimize the comfort and health of patients and their families in their home environments. This role includes patient education tailored to individual needs and values, guided by an interdisciplinary plan of care. The RN must practice professional nursing in compliance with the NH State Nurse Practice Act. This full-time exempt position requires availability from Friday at 4:30 pm through Monday at 8:00 am, including on-call responsibilities and home visits in the Littleton and surrounding area.

Requirements

  • Graduation from an accredited school of professional nursing.
  • Current Registered Nurse (RN) license in the state of New Hampshire.
  • Association accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date.
  • Proficiency in oral and written communication.
  • Strong interpersonal skills.
  • Excellent organizational and time management capabilities.
  • Ability to work independently in a home care setting.

Nice To Haves

  • Previous clinical experience in a healthcare setting is preferred but not mandatory.

Responsibilities

  • Conduct comprehensive assessments of patients in their home settings.
  • Develop, implement, and evaluate individualized nursing care plans.
  • Provide education to patients and families tailored to their needs and environments.
  • Administer medications and treatments as prescribed and in accordance with care plans.
  • Collaborate with interdisciplinary teams to ensure coordinated and comprehensive care.
  • Document patient assessments, interventions, and responses in a timely and accurate manner.
  • Manage and prioritize multiple patient visits and on-call responsibilities effectively.
  • Offer emotional support to patients and families, respecting their values and preferences.
  • Adjust care approaches and plans based on patients' evolving conditions and feedback.
  • Ensure all care provided meets NH State Nurse Practice Act standards and agency policies.
  • Maintain open and clear communication with patients, families, and healthcare team members.
  • Respond promptly to patient or family emergencies and adapt care accordingly.
  • Conduct patient and family teaching sessions, promoting health maintenance and disease prevention.
  • Participate in ongoing professional development and competency evaluations.
  • Utilize organizational resources efficiently to support patient care and agency operations.
