About The Position
Empath Home Health is growing! We're expanding our home health services in Pasco County and looking for a passionate Weekend Baylor RN (Home Health) to join our team. This is an exciting full-time opportunity to be part of a market expansion, serving patients the comfort of their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the community. Locations: home health visits in Pinellas county and Pasco county. Visits strategically scheduled based on location and need. Schedule: This is a Weekend Baylor role requiring full weekends. You decide which schedule fits your life! 3 days (Friday-Sunday OR Saturday-Monday) 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday) Why join us? Requirements
- Active Florida RN license
- Minimum of 1 year of direct patient care experience required
- Strong assessment, critical thinking, and patient education skills
Nice To Haves
- BSN or higher degree preferred
- Home health RN experience strongly preferred
- HCHB (Homecare Homebase) EMR experience strong preferred
Responsibilities
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Provide nursing services and/or treatments indicated on plan of care.
- Counsel and educate the patient and family on disease process, self-care techniques, and prevention strategies.
