About The Position

Puget Sound Home Health & Hospice provides compassionate and holistic hospice care to patients and families across Pierce County. We are seeking an experienced Hospice Admissions Registered Nurse (RN) to join our team in a weekend on-call role focused on patient admissions and urgent visits. Requirements

Active Washington State RN license (or compact equivalent).
Hospice admissions and on-call experience required.
Proficiency with HCHB (HomeCare HomeBase) EMR required.
Strong clinical assessment, organization, and communication skills.
Compassionate and composed under pressure; able to manage emotional patient and family interactions.
Reliable transportation, valid driver’s license, and current auto insurance.
Must be available to work Friday 5:00 PM – Monday 8:00 AM PST .

Responsibilities

Serve as the primary on-call admissions nurse during weekends, performing initial hospice assessments and completing required documentation.
Conduct in-home or facility-based admissions, evaluations, and symptom management visits as needed.
Provide expert clinical judgment, emotional support, and education to patients and families during the admission process.
Communicate and coordinate with the hospice interdisciplinary team (physician, social worker, chaplain, aides) to ensure timely and compassionate care.
Maintain accurate and prompt documentation in the EMR system.
Respond to urgent calls and coordinate follow-up care as appropriate.
