About The Position

The Weekend Admissions Nurse is a per diem position, working 16 hours over the weekend, and we are looking for 2 individuals to alternate weekend coverage. This position is responsible for assessing patient needs and coordinating admissions during weekends to ensure seamless care delivery. Specific tasks include:
- Admits patients to service for Hospice and Home Health Care
- Does evaluation for perspective hospice patients as needed
- Educates patients and families about hospice services and symptom management
- Performs assessment of patient and family, through observation, interviews, and analysis of records.
- Analyzes physical, emotional, psychosocial, environmental, and spiritual needs.
- Identifies past, present, and potential problems
- Develops a comprehensive care plan
- Makes routine visits to patients as needed

Requirements

• Graduation from an accredited school of nursing with a Bachelor of Science in Nursing degree.
• Registered Professional Nurse with a current license in the State of Connecticut.
• Minimum of 2 years' experience as a staff nurse in a hospital and/or home health care agency and at least 1 year in hospice.
• Valid, clean driver's license & transportation required (may be eligible for an agency vehicle if available)
