About The Position

The Weekend Admission Nurse position is per diem and works 16 hours on the weekend. We are looking for 2 individuals to be able to rotate weekends. Work location is on the road to private residences, skilled nursing facilities, assisted living facilities, group homes--wherever the patients currently reside. The Weekend Admission Nurse is responsible for assessing patient needs and coordinating admissions during weekends to ensure seamless care delivery. Specific tasks include: - Admits patients to service for Hospice and Home Health Care - Does evaluation for perspective hospice patients as needed - Educates patients and families about hospice services and symptom management - Performs assessment of patient and family, through observation, interviews, and analysis of records. - Analyzes physical, emotional, psychosocial, environmental, and spiritual needs. - Identifies past, present, and potential problems - Develops a comprehensive care plan and communicates to attending physician and other staff members -Completes paperwork required by agency, government, and third-party payer systems. - Makes routine visits to patients as needed

Requirements

  • Graduation from an accredited school of nursing with a Bachelor of Science in Nursing degree preferred.
  • Registered Professional Nurse with a current license in the State of Connecticut.
  • Minimum of 2 years' experience as a staff nurse in a hospital and/or home health care agency and at least 1 year in hospice required.
  • Valid, clean driver's license & transportation required (may be eligible for an agency vehicle if available.)

Nice To Haves

  • Prior admissions experience helpful!
  • Certified Hospice & Palliative Nursing credential desired.
