About The Position

We're excited to announce that a new PACE (Program of All-Inclusive Care for the Elderly) center is coming to Somerset County and we're building a team of compassionate, skilled professionals to help bring this life-changing model of care to life. PACE is a nationally recognized program that helps seniors live safely and independently in their homes and communities, while receiving comprehensive, coordinated care. Through an interdisciplinary team approach, PACE provides medical services, social support, therapy, home care, and more, all tailored to the unique needs of each participant. Requirements

• Active and valid state licensure or certification (as applicable)
• Strong communication and interpersonal skills
• Commitment to compassionate, patient-centered care
• Ability to work independently and as part of a dynamic care team
• Passion for serving older adults
