The Waiver Facilitator is responsible for acting as the primary point of contact for Pawnee’s clients who utilize or qualify for the State of Kansas Home and Community Based Services (HCBS) Severely and Emotionally Disturbed (SED) waiver. They are responsible for tracking services and other clerical responsibilities to meet Kansas requirements and are a subject matter expert on the program.

Requirements

  • Bachelor’s degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.
  • Strong oral and written communication skills using the English language.
  • Basic personal computer skills.

Responsibilities

  • Comply with requirements of the State of Kansas Home and Community Based Services (HCBS) Severely and Emotionally Disturbed (SED) waiver.
  • Assist in the assessment of HCBS SED waiver referrals to determine the level of waiver services needed to prevent a higher, more restrictive, level of care.
  • Establish an initial written Plan of Care (POC) that includes access to informal community-based resources, the child and family’s strengths and needs, treatment goals, objectives and actions steps, and a crisis plan that will help the child succeed in the community. Update the POC on the family choice date annually.
  • Monitor current services, child and family/caretaker needs and service outcomes while the child is enrolled in the HCBS SED waiver to determine if services received are as prescribed in the current treatment plan/Plan of Care.
  • Promote collaboration among family, caretakers, service providers, educators, and community resources to ensure that appropriate and effective services are delivered to the child and family/caretaker.
  • Ensure a Child Behavior Checklist (CBCL) has been completed and updated semi-annually with the scoring information being documented in the clinical chart.
  • Utilize the State of Kansas data entry system to track and submit required HCBS SED waiver data.
  • Ensure the tasks and documentation outlined in each client’s Plan of Care are completed so service authorizations are maintained. Track these activities on the waiver spreadsheet.
  • Review Automated Information Mapping System (AIMS) data and collaborate with Youth Rehabilitation Services for completion and submission to the State.
  • Provide support and consultation to staff, community, and clients about the process for HCBS SED waiver application and eligibility.
  • Attend clinical staff meetings to monitor and track the child’s progress with the Plan of Care and additional resources or services that are necessary.
  • Maintain and advance knowledge and skills related specifically to position or to field of work through regular review of pertinent materials or participation in training events as approved by supervisor.
  • Adhere to the standards, policies and instructions established by PMHS Policy and Procedure and any applicable ethical codes within field of employment.
  • Review and comply with the Pawnee Code of Conduct outlined in the Employee Handbook.
  • Perform other reasonably related duties as assigned by immediate supervisor or other management personnel.
